- Purchase any eligible Colgate PerioGard product
- Scan your physical PC Optimum card in-store at Shoppers Drug Mart/Pharmaprix or enter your card number if purchasing online
- You will receive one (1) entry per eligible transaction
No Purchase Necessary (NPN)
- Handwrite the following information on a piece of white paper: your first name, last name, phone number, date of birth, and PC Optimum account number
- Include a brief text explaining which Colgate PerioGard product you wish to try and the reason why
